What happens to my records if I move to another practice?

Your records are held securely by Livelong.

If you would like them transferred to another practice, simply let us know and complete a consent form. We will handle the process for you.

Can I get a copy of my notes?

Yes.

You have the right to request a copy of your records at any time. Just contact the practice team and we will arrange this for you.
